5 Conditions Where the Activator Method Is Often the Better Fit

In my last article, I compared the Activator Method and traditional chiropractic side by side. The honest takeaway was that both approaches help a lot of people, and the small trials comparing them have found patients often improve similarly. You can read that full comparison in Activator Method vs. Traditional Chiropractic: Which Is Right for You?

But “similar results on average” doesn’t mean every approach fits every patient equally.

Some conditions make high-force, twisting adjustments uncomfortable, impractical, or simply inappropriate. For those patients, a gentle, precise, low-force approach isn’t just a preference. It’s often the difference between getting chiropractic care and going without it.

After nearly 50 years in practice, here are the five conditions where I see the Activator Method make the biggest difference.

5 Conditions Where the Activator Method Is Often the Better Fit

Why the Method Matters for Certain Conditions

Traditional chiropractic adjustments typically involve positioning the patient, often with rotation of the neck or a side-lying twist of the lower back, followed by a quick, forceful thrust. For a healthy spine, that’s well tolerated by most people.

The Activator Method works differently. A small handheld instrument delivers a quick, controlled impulse to a precise point on the spine. It fires faster than muscles can react, so your body doesn’t tense up against it. There’s no twisting, no cracking, and no high-velocity manipulation. The instrument’s force can also be adjusted to suit the patient.

That combination of precision, low force, and stable positioning is what makes the Activator Method such a good fit for the conditions below. You can learn more on our Activator Method page.

1. Herniated and Bulging Discs

When a disc herniates, the muscles around it tighten to protect it. That guarding is your body telling you not to move the area. Twisting a spine through that protective tension is exactly what many disc patients fear, and it’s often the reason they’ve avoided chiropractors altogether.

Why the Activator Method Fits

The Activator Method delivers a targeted adjustment without rotating or loading the injured disc. Patients stay comfortably positioned, and the impulse is too quick for the guarding muscles to resist. Most of my disc patients tell me their first visit was far more comfortable than they expected.

Every disc case starts with a thorough neurological exam and a review of any imaging. I’ve completed more than 1,700 hours of post-graduate training in X-ray, and we track reflexes, strength, and sensation throughout care so we’re measuring real progress.

2. Failed Back Surgery Syndrome and Post-Surgical Spines

Some patients come to me after spinal surgery, still in pain. It’s common enough to have a name: Failed Back Surgery Syndrome. These patients have often been told there’s nothing left to try.

A spine with fused segments, implanted hardware, or surgical scar tissue should never be twisted. That alone rules out many traditional adjustments for post-surgical patients.

Why the Activator Method Fits

The Activator Method lets us work around a fusion with precision. After a fusion, the segments above and below often take on extra stress, a well-recognized problem known as adjacent segment degeneration. A precise, low-force instrument allows us to address those neighboring segments without disturbing the surgical site.

Failed Back Surgery Syndrome is one of my areas of focus. We review your surgical history and imaging carefully and coordinate with your surgeon or medical team when needed. 3. Whiplash and Auto Accident Injuries

After a car accident, the soft tissues of the neck are often sprained, inflamed, and guarded. Patients are sore, stiff, and understandably wary of anyone turning their head. Even a low-speed rear-end collision in a Sioux Falls parking lot can cause real injury.

Why the Activator Method Fits

Fresh injuries need care that doesn’t aggravate them. The Activator Method allows treatment to begin without rotating an injured neck or forcing motion through inflamed tissue. That means patients can start care sooner, which matters both for recovery and for documenting the injury.

I’ve completed the Whiplash Masters Program along with post-graduate training in cervical trauma, accident reconstruction, personal injury, and AMA impairment rating. We document injuries thoroughly, work with attorneys, and know the claims process cold. We accept PIP, MedPay, and at-fault liability claims.

4. Neck Pain and Headaches

The neck is where people are most nervous about chiropractic care. I hear it constantly: “I’m fine with my back, but please don’t crack my neck.” For many people, that worry alone has kept them from getting help for chronic neck pain or headaches that start in the neck.

Why the Activator Method Fits

With the Activator Method, we never rotate or crack the neck. Patients stay comfortably positioned while the instrument delivers a precise impulse to the exact area that needs attention. Because there’s nothing to brace against, even patients with long-standing fear of neck adjustments relax quickly.

We evaluate the whole spine, not just the area that hurts, because neck pain and tension-type headaches often involve more than one region. Learn more on our neck pain page and our headache and migraine page.

5. Osteoporosis and Age-Related Spinal Changes

As we age, bone density often decreases, discs lose height, and arthritis changes how joints move. Most clinicians approach high-force adjustments with caution in patients with significant bone loss. Unfortunately, that sometimes means older adults are told chiropractic care isn’t an option for them at all.

Why the Activator Method Fits

A low-force, adjustable approach lets older adults receive chiropractic care without the strain of high-velocity adjustments. Patients stay in a stable position, and we can adjust the instrument’s force to match each person’s needs. Many of our patients are in their 70s, 80s, and beyond.

We take a thorough health history, including any bone density results, prior fractures, and medications that affect bone health. If anything suggests a different kind of care is needed first, we’ll tell you and refer you appropriately.

Other Conditions Where Gentle Care Makes Sense

These five are where the difference is most obvious, but they aren’t the only ones. The Activator Method’s precision and comfort also make it a strong fit for:

  • Everyday low back pain, especially when it’s acute and guarded. Learn more about our low back pain care.

  • Work injuries, where early, comfortable care helps people get back on the job. Learn more about our work injury care.

  • Pregnancy-related back pain, where comfortable positioning and low force matter. Learn more about prenatal chiropractic.

  • Children, whose growing spines call for a gentle touch. Learn more about chiropractic care for kids.

Why Experience With the Method Matters

Any technique is only as good as the doctor using it. Both my son, Dr. Sean Hagen, and I are Advanced Proficiency Rated in the Activator Method, the highest credential available through Activator Methods International. I’m one of only five doctors in the Sioux Empire with that rating, and together we’re the only father-son Advanced Proficiency Rated Activator team in the region.

Back Specialists is the only 100% Activator Method clinic in Sioux Falls. We don’t switch techniques depending on the patient or the day. Every adjustment, for every condition, uses the method we’ve devoted our careers to.

When We’re Not the Right Fit

We’re not the right fit for everyone. Some conditions need medical evaluation before any chiropractic care, including signs of spinal cord or nerve compression, unexplained weight loss, fever with back pain, or a recent significant trauma. If we see anything during an exam that calls for a different kind of care, we’ll refer you the same day. I wrote more about this in Chiropractic Red Flags.

Frequently Asked Questions

Is the Activator Method better than traditional chiropractic?

Not for everyone. Small studies comparing the two have found patients often improve similarly. Where the Activator Method stands out is comfort and safety for patients who shouldn’t or don’t want to have high-force, twisting adjustments.

What conditions does the Activator Method treat?

At Back Specialists, we use the Activator Method for low back pain, neck pain, headaches, sciatica, herniated discs, whiplash, work injuries, Failed Back Surgery Syndrome, and more. We also care for expecting mothers and children.

Can I see an Activator chiropractor if I have osteoporosis?

In many cases, yes. The Activator Method is low-force, and the instrument’s force can be adjusted. We review your health history, bone density results, and any prior fractures first, and we’ll refer you if another kind of care is needed.

Is the Activator Method safe after a spinal fusion?

For many post-surgical patients, yes. We never twist a fused spine. The precision of the Activator instrument lets us care for the segments around a fusion without disturbing the surgical site. We review your surgical history carefully before starting care.

Can the Activator Method help with headaches?

Many headaches, especially tension-type headaches, involve the neck and upper back. The Activator Method lets us address those areas without rotating or cracking the neck. Your exam will help determine whether your headaches are likely to respond to chiropractic care.

Find Out if Gentle Activator Care Is Right for You

If one of these conditions has kept you from trying chiropractic care, or you’ve been told chiropractic isn’t an option for you, I’d be glad to take a look. We’ll do a thorough exam and give you an honest answer about whether we can help. If we’re not the right fit, I’ll tell you that too.
